details of how to access care. NHS patients needing urgent
treatment outside of normal
opening hours should call 111.

ACCESSIBILITY
Unfortunately, we are unable to offer access to patients
with wheelchair or mobility needs due to the nature
of our premises.
COMPLAINTS
If you have any concerns with your treatment, service that
has been provided or comments of any kind, please speak
to our Practice Administrator.
Alternatively, you may wish to contact the NHS complaints
service or the Dental Complaints Service.
FEEDBACK
We would welcome your feedback to improve the service
we offer. Please complete a feedback form or
speak to a member of staff.
CANCELLATIONS
We require 24 hours notice in the event of an
appointment cancellation (subject to illness or
exceptional circumstances). We reserve the right to
charge for missed appointments or late cancellations.
