COVID-19 UPDATE
In order to ensure patient and staff safety we would request the following when attending the practice:
If you have symptoms of COVID-19 or a member of your household/family have symptoms we would ask you to delay your appointment unless it is an emergency.
A COVID-19 screening form will be sent to you prior to your appointment.
When you arrive at the practice a further COVID-19 screening will take place.
Please use hand sanitiser on arrival and exit.
Please socially distance and wear a mask in the waiting room (unless exempt).

OUT OF HOURS ARRANGEMENTS
Please call the practice and the
answering machine will provide
details of how to access care. NHS patients needing urgent
treatment outside of normal
opening hours should call 111.

ACCESSIBILITY
Unfortunately, we are unable to offer access to patients
with wheelchair or mobility needs due to the nature
of our premises.
COMPLAINTS
If you have any concerns with your treatment, service that
has been provided or comments of any kind, please speak
to our Practice Administrator.
Alternatively, you may wish to contact the NHS complaints
service or the Dental Complaints Service.
FEEDBACK
We would welcome your feedback to improve the service
we offer. Please complete a feedback form or
speak to a member of staff.
CANCELLATIONS
We require 24 hours notice in the event of an
appointment cancellation (subject to illness or
exceptional circumstances). We reserve the right to
charge for missed appointments or late cancellations.
